| Step 1: | Carefully review the Disclosure and Dataset Use Agreement on the next page. If you agree to its terms, print a hardcopy, sign it, and snail-mail it with your completed application form to K. Warner Schaie Please note that you will not be granted dataset access privileges until a signed, hardcopy of the Disclosure and Data Use Agreement is received by our office. |
| Step 2: | After you have printed your hardcopy of the Disclosure Agreement, click “I Agree” which will link you to the new user application form. By first presenting the Agreement terms, we hope to save you time from completing the user application form if you are unable to comply with the terms of dataset use. |
| Step 3: | Complete all required fields of the on-line User Account Application, click on Print, and mail the hardcopy to us at the address in Step 1. You will receive a reply via email that your form has been delivered and is under consideration. |
| Step 4: | Once you have been approved as a dataset user, you will be provided an user id and password via e-mail. The authorization process may require several working days and you may be requested to submit additional information for review by the Principal Investigators. |
